1. Log into Logos

  2.  Click on “Forms” on the navigation items on the left

  1.  Click “New form”

  1.  Complete the fields: (and click save)

  1.  Navigate your way back to the “Forms” page, as per step 2

  2.  Click the edit icon

  1.  Insert destination email addresses - You are able to add 3 email addresses 

  1.  Click on “Form Elements”

  1.  From this page, you are able to add the elements that the form will be build from. You will see an auto-created form with the first 3 options names “First Name,” “Last Name,” and “Email.” *THESE ARE MANDATORY FIELDS, DON'T CHANGE OR THE FORM WON'T WORK*

  2.  To add a field to the form you must complete the form, paying close attention to the details besides each input.

  3. Once you have created the form you will need to create a themed content chunk, to do so click on “Content Chunks”

  1.  Click “New Site Content Chunk” 

 

  1.  Under the title field enter the same title as earlier (example Test_form) and copy and paste the following code into the Content field, but edit the highlighted to the name of the form. PLEASE NOTE: Each form name must only be used once, so if you wanted to add another form, it is important that they are named differently. 


BEFORE Copying and Pasting inro the Content field, make sure “Source” is clicked:



Proceed to copy and paste the following code:

Edit the highlighted to the name of the form Test_form so the code will look like:

Click SAVE


  1.  Now we must assign this form to the relevant page/article. Once you have decided where you would like this form to be placed, you must copy and paste the following code into the WYSIWYG editor:

Similarly editing the code to match the name of the content chunk you have just created, to make it easier to create, naming the content chunk and the form tends to make the process a lot easier to manage.
